Влияние обеспеченности онкологами на смертность населения от новообразований в регионах России

Аннотация
Как определить оптимальное количество онкологов в регионе? Помимо прочего, актуальность исследования определяется наблюдаемой вариацией этих показателей в регионах РФ. В частности, в 2021 г. наблюдался разброс соответствующего показателя смертности онкологических заболеваний от 54 на 100 000 человек в Республике Ингушетия до 270 на 100 000 человек в Тульской области, при этом разница в уровне обеспеченности врачами-онкологами от региона к региону может достигать четырех раз. Целью исследования является моделирование влияния уровня обеспеченности онкологами на показатель смертности от новообразований в регионах РФ в контексте определения оптимального количества профильных врачей. Авторские гипотезы предполагают недооценку отрицательного влияния врачей на показатель смертности ввиду существования ложноположительных связей между рассматриваемыми переменными, а также существование убывающей отдачи от найма дополнительных врачей по мере роста уровня обеспеченности профильными кадрами. Дизайн исследовательского проекта предполагает регрессионный анализ на основе квазиэкспериментального метода инструментальной переменной для решения проблемы обратной причинности и пропущенных переменных с помощью моделей на основе пространственной выборки и панельных данных с последующим выводом функции, описывающей прирост числа спасенных жизней в случае найма дополнительных врачей. Согласно полученным результатам, в среднем увеличение уровня обеспеченности онкологами на 1 % приводит к снижению смертности от новообразований на не менее чем 0,4 %. При уровне обеспеченности врачами-онкологами в количестве 1 на 100 000 человек их предельный вклад в количество спасенных жизней составляет не менее 96 на 100 000 человек с дальнейшим снижением на 14 человек по мере роста уровня обеспеченности на 1 единицу. Результаты работы могут быть использованы при определении оптимального числа врачей-онкологов на территории, а также при принятии решений о распределении средств между наймом врачей разных специальностей или другими затратами в условиях ограниченных ресурсов.
